Ultrasound, Procedure and Consult Requests

To schedule a mobile ultrasound and procedure, or a consult, please click the appropriate button below to submit a request. In general, I will respond within 1 business day of receiving a request form. For urgent cases, please call and leave a message about the nature of the case, and complete the form as thoroughly as possible. I will return your call as soon as I am able. Please note, I am not always available for emergencies, and I will make every effort to help when I can.

In addition to the request form, please send all pertinent medical records via email or fax. Texted pictures are good as well. I will review x-rays, still ultrasound images, ecg strips, written and electronic medical records, pictures of lesions, etc. Also any reports from radiologist, pathologist, etc. are needed as well.
